Acceptance into Caritas

Referrals can come from Doctors, Rns, discharge planners, clergymen, friends, family, public health or the patient.

Criteria for acceptance into CARITAS:

  • Patient is seriously or terminally ill and attending physician approves of CARITAS involvement
  • Patient and family want CARITAS involvement
  • Client lives in service area
  • Client is aware of diagnosis

Step 1: Referral to Caritas

If you or someone you know needs assistance, simply call the Caritas office at 798-6648.

Step 2: The Home Visit

The Caritas Program Assistant or Executive Director will visit your home to assess your needs and to ensure that Caritas can provide the appropriate support and services for you and your family. In addition, suitable volunteers will be considered and any questions you may have about Caritas will be answered. If it is determined during the Home Visit that another agency and its services would be more appropriate to meet your needs, a referral will be recommended and made by Caritas, subject to your approval.

Step 3: Volunteer Visits Begin

The volunteer assigned to you will begin visiting you. Case update meetings will be held regularly to determine that your needs are being met.
